In the Mouth of Madness (1995)

Scene: Sam Neill's character wakes up to a chaotic world as his door is gorged open and the other mental asylum residents are shredded to pieces, still dressed in the issued scrubs that he has altered by now with drawings of crucifix, he roams the post-apocalyptic streets, and finally goes into a deserted movie theater playing "In the Mouth of Madness". To make matters even more personal, he is in the starring role playing himself of course; with popcorn in his hands, he laughs hysterically at the screen.

 Bottom Line: In the Mouth of Madness has so many unsettling scenes ie the granny who was tormenting her husband with her tentacles and finally axing him to death, Sutter Cane ripping his face and entire body off like a page from a book, and the infamous aging bicycle rider  whom Julie Carmen's character kept passing multiple times throughout the night drive to the fictional town, Hobb's End, but the scene that got me spooked the most was the final one because it leaves room for you to wonder if the movie was just a big mindfuck about a fictional character who believes he is real, a delusional man who is scared by a nighmare-giving book and author or if any of John Trent's misadventures in Hobb's End was ever real or purely the result of his demented imagination. If you get scared easily, you should watch 'In the mouth of Madness' in plain daylight with lots of friends as this movie is guaranteed to give you daymares on top of other things for awhile. This premise was part of John Carpenter's Apocalypse Trilogy with the other two films being "The Thing" and "Prince of Darkness".
